This book report and summary form is detailed and elicits complete information from the user. It asks for specifics about the book, i.e. copyright date, setting, characters, genre, etc. It also asks the reader for predictions after just reading a few pages of a book. The form appears to be designed to be used to monitor understanding in an ongoing manner. The reader is asked to reflect on cause and effect, compare and contrast, fact and opinion. The reader is asked to list parts of speech, sequence events and use them to write a summary, identify author’s purpose, make generalizations, visualize, predict, draw conclusions and interpret the theme the author is trying to teach. The reader is also asked how the theme can be applied to the reader’s life, or aid the reader in building empathy toward someone else’s life. The form ends by asking the reader to examine his or her self-knowledge.
