This is my daily formative assessment template that I give at the end of class. Each day I ask one content question (a) and 1 Math Practice question (b). The first Math practice question is just a check off box to remind students of the practices they should be trying. The rest of the time I might ask something like "Explain how you personally persevered today." or "How did you use MP 2 today?" This informs my instruction daily AND helps students with metacognition around those important Math Practices. I like to keep the exit tickets all in one place on this template so the students can use them to review for summative assessments - or I can use it to inform parents of class engagement, etc. Please feel free to modify and let me know what you add or how it works for you!
